CASE MANAGEMENT FOR THE FRAIL ELDERLY

Case Management is a program of coordinated community services specifically targeted to older Iowans.

HOW DOES IT WORK?


Financial eligibility is determined through DHS.
A visit will be arranged in your home.  You will be asked
questions to assess your needs.  The interviewer will ask
your permission to discuss the informtion with a team of people
from service agencies in order to coordinate the assistance
you need.  One person on that team will be
your primary helper or Case Manager.  There
are choices for your case manager.  Your Case
Manager will arrange for the help you need and will continue
to be available as other needs arise.  You and your family
decide on any services you receive.
 


WHO CAN QUALIFY?


 

Persons who meet all of the following criteria:

Age 65 or older
Who need nursing home level of care
Meet income and resource guidelines
Receive case management services through the
Case Management Program
for the Frail Elderly coordinated
by the Area Agency on Aging.


 


 
WHAT ARE THE ADVANTAGES?


The individual and his/her family will have informed choices about services in the community.
Family and in-home services will be encouraged.
When agencies work together, they can be sure that the individual is getting all the help needed without overlap or gaps.
The individual will only have to call one perons, the Case Manager, instead of several agencies or organizations.


WHAT DOES IT COST?

There is no cost to participate in the Case Management Program.
